In 1995 the Borough of Caldwell combined its Health, Welfare, and Senior Services Departments under one umbrella of Human Services.  Since then, the Department of Human Services is responsible for the day-to-day operations of each division including the Caldwell Health Department, Office of Vital Statistics, the Grover Cleveland Senior Center, the Caldwell West Caldwell Roseland Senior Transportation Program, and the Caldwell Food Pantry.

The Caldwell Health Department

Office of Vital Statistics:

  • Certified Copies of a birth, marriage or death certificate that occurred in Caldwell are available.  To obtain a certified copy, you must fill out an application that is listed under the Municipal Forms section labeled “Vital Statistics”.  The cost for a certified copy is $10 and must accompany the completed form and valid ID. Please note you must have a relationship to the document in order to obtain a certified copy of that document.

  • Marriage Applications are done by appointment only.  In order for you to complete the marriage application within the Borough of Caldwell, there is a residency requirement for applicants.  However, if both applicants live outside of the State of New Jersey and the marriage will take place within Caldwell, you would apply in the Borough of Caldwell.  To complete the marriage application, the two applicants must appear in person with a witness who is over the age of 18 with a picture ID.  Both applicants must provide proof of residency and a picture ID.  If someone was married previously, you must show proof of how that marriage was dissolved.  The cost for the license is $28 payable in cash or check.  Please call the Health Department directly to arrange for an appointment at least two weeks before your wedding.
